Before you take it

  • No NIH ODS Tolerable Upper Intake Level (UL) has been established for berberine; clinical trial data supports doses up to approximately 1.5 g/day for up to 6 months, but long-term safety data beyond 6 months is not well characterized.
  • Potent quasi-irreversible inhibitor of CYP2D6 (approximately 9-fold suppression) and inhibitor of CYP3A4 and CYP2C9; raises clinically significant drug interaction risk with a large portion of prescription medications whose metabolism depends on these pathways.
  • Contraindicated in pregnancy (crosses the placenta; associated with neonatal jaundice and kernicterus risk in newborns) and during breastfeeding.
  • Additive hypoglycemia risk when co-administered with antidiabetic agents including metformin, insulin, and sulfonylureas.
  • Commonly reported gastrointestinal adverse effects in clinical trial participants include nausea, constipation, abdominal discomfort, and diarrhea.
  • Antibiotic-like modulation of gut microbiota composition, including potential suppression of beneficial species; long-term microbiome consequences in humans are not established.
  • Berberine produces bidirectional effects on bowel function — constipation in some users and diarrhea in others in the same clinical trials — meaning individual response for 'irregular digestion' is unpredictable and the supplement may worsen the symptom it is intended to address.
