Before you take it

  • No Tolerable Upper Intake Level (UL) for LGG or probiotics has been established by NIH ODS or the Institute of Medicine; no supplemental dose ceiling applies.
  • Bacteremia risk in immunocompromised and critically ill patients: A retrospective analysis of 22,174 ICU patients (NIH ODS cited) found 6 genome-confirmed Lactobacillus bacteremia cases among 522 LGG recipients versus 2 among 21,652 non-recipients. Additional case reports document endocarditis and abscess formation in transplant and other immunocompromised patients. LGG should be used with caution or avoided in these populations.
  • Antibiotic timing: Probiotic viability is reduced by co-administration with antibiotics. NIH ODS guidance is asymmetric: take LGG at least 2 hours before an antibiotic dose, or 4-6 hours after dosing. Starting within 2 days of the first antibiotic dose is recommended for prevention of antibiotic-associated diarrhea.

The detail

The paper has three problems together. All the patients were children. What it measured was how often and how badly their stomachs hurt — it contains no data on stool at all. So it cannot support a claim about irregular digestion in adults.
